DENIAL OF SERVICE - INCORRECT INFORMATION
ADOPTED: JUNE 29, 1995

A library card may not be used until the library shall determine that sufficient and correct address information has been supplied to the library.

It is the responsibility of the patron to supply the library with updated information regarding the individual's name and address. Failure to supply correct information will result in denial of service.

Service will be restored when the patron supplies proper documentation of correct address and name, provided there are no other restrictions in effect causing service to be denied, i.e., over due materials.

DENIAL OF SERVICE - APPEAL

The following policy shall constitute right of appeal for denial of library services by the Jefferson County Library.

Citizens who have been denied library service for any reason shall, after the normal course of review by the branch or extension manager as specified within various policies of the District, have fifteen days to appeal in writing such denial to the Director of the Library District. The Director shall respond in writing with a ruling concerning the denial within fifteen days.

If the Director finds that service shall continue to be denied, a final appeal may be made to the Jefferson County Library Board of Trustees. Such appeal must be made in writing and must arrive at District's Administrative Offices (Central Services) no later than the 1st of the month for inclusion in the Current month's board packets. The Trustees shall be supplied with the final patron appeal along with all findings by management. Appeals to the Board of Trustees must be filed within thirty days of the date of the denial finding submitted by the director.
