1)  All library card applicants must provide proof of identity. Such proof may be one of the following:

• Current Driver's License
  
• Current State-Issued Photo Identification Card
  
• Birth Certificate
  
• Current Student ID with Photo
  
• Current Military ID
  
• U.S. Passport
  
• U.S. Visa
  
• U.S. Diversity Visa (Green Card)

2) All applicants for Resident (free) library cards must provide proof of legal residence or ownership of real estate within the boundaries of JCL's active subdistricts plus proof of current address. Applicants for NonResident (annual fee) library cards must provide proof of current address. Such proof may be one of the following:

• Current Driver's License

• Current State-Issued Photo Identification Card

• Current Voter Registration Card

• Current Missouri Tax Receipt showing address

• Current Year Real Property Tax Receipt or Jefferson County or City Tax Assessor's Verification

• Current Vehicle Registration showing address

• Current Missouri Automobile Insurance ID Card showing address

• Current Telephone or Utility Bill indicating applicant's name or address

• Bank Printed Checks or Bank Statement Personal Account showing current address

• Current Lease Agreement, Rent Receipts or Occupancy Permit showing address

3) Complete the application card and sign it. Applicants who are age eighteen and older assume responsibility for fines or other charges incurred while using the library. A parent or legal guardian must sign the application for minors under the age of eighteen.
